Your First Office Visit

Your initial office appointment will consist of a consultation with Dr. Chionchio to evaluate you and determine your diagnosis and treatment options. Occasionally, procedures such as extractions can be performed the same day as the consultation. However, a complex medical history or treatment plan will require an evaluation and a second appointment to provide treatment on another day. Patients requesting or requiring IV sedation or general anesthesia require a pre-operative evaluation except in certain emergency situations.

X-Rays: If your dentist or physician has taken x-rays, you may request that they forward them to our office. If there is not enough time, please pick them up and bring them to our office. Even though you may have recent routine dental films, we frequently need to take a special x-ray called an orthopantomogram or panorex to more carefully evaluate your teeth and the supporting jawbone. If additional films are necessary, we will take them at our facility.

It would help save you time on your first visit
if you would please fill out our Patient Health History Form

  4. Any current X-rays if they were given to you.
  5. A list of medications you are presently taking (i.e. heart medications, aspirin, anticoagulant therapy, etc)with the Doctor Information (Who prescribed the medications).
